i had both my medicine and surgical posting exams todae !!!
yes, 2 exams in a dae !!!
medicine one in the moring and surgical one in the afternoon..

the medicine posting exam was HORRIBLE !!!
i just hope i pass :(
i hate my patient.. idiotic madam Ho !!!
she's a SABO QUEEN lor !!

here's wad happened (summarised version)
i was supposed to take a history from this lady called madam Ho..
madam Ho presented with pain in her left loin region..
so anywae, after she told me bout the pain..
i tried to ask her bout the 10 aspects of her pain..

oh anywae, madam Ho is an english speakin patient..
she told me that she preferred english to chinese..
and no, i did not offend her..
we even made small talk bout medicine life before i started my history taking..

kerly: so madam Ho, when did your back start to feel pain?
madam Ho (MH): actually got pain long ago, but this 2 weeks more painful..
kerly: so this pain started long ago? how long ago?
MH: hmm, very long ago lor..
kerly: hmm, how long ago? 1month ago? 6months ago? 1year ago?
MH: hmm, dunno leh, very long ago leh..
kerly: so you dun rem when it started is it ? maybe roughly? bout 1year ago? or 5years ago?
MH: hmm, i think its quite long ago lah.. dunno..

kerly: so madam Ho, you said your loin pain, is there anything you do that aggravates it? hmm, like make it more painful?
MH: oh yar, when i bend down and touch my toes lor..
kerly: oh, so when you bend down and touch your toes den more painful is it?
MH: yes, den sometimes i lie down also quite pain lah..
kerly: oh, den is there anything you do that makes it less painful?
MH: yar yar, of course lah.. just dun bend down and touch my toes lor...
kerly: oh, i mean, is there anything you do, like maybe stand up or walk around or eat something den less pain?
MH: yar lah.. just dun bend down den can lor.. when dun bend down den not painful wad..
kerly: hmm, how bout when you sit up, is it less painful?
MH: aiyo, all the same.. only when i bend down den pain.. if dun bend down, den not so pain

kerly: okae, so madam Ho, are you on any medication for your pain?
MH: nope..
kerly: no nothing? did you eat panadol on your own or anything?
MH: nope, never..
kerly: so, you're not on any medication.. did the doc give you any medicine, like painkillers or anything?
MH: nope, i'm not on any medication right now..

kerly: so madam Ho, this pain of yours.. is it only at your back? or did it spread to other parts of your body?
MH: hmmm..
kerly: maybe spread to your stomach or to the other side of your back??
MH: oh yar yar, it spreaded to my stomach here (point to her left lunbar region) and sometimes here (umbilical region)
kerly: oh, so here (left lumbar region) and here (umbilical region) . how bout your arms? or your back? or other parts of your body?? did it spread o anywhere else?
MH: nope, only spread to these areas ...
kerly: oh, so only here and here?
MH: yes..

den the doc came in and started to question me bout madam Ho..
by the wae, madam Ho was present when he questioned me..
so i couldnt defend myself at all...

doc: so, when did madam Ho's pain started?
kerly: erm, i'm not very sure as madam Ho could not rem when it started exactly.. but it is a chronic pain which started quite long ago.. and it got worst within this 2 weeks..
doc: really? did you ask her?
kerly: yes, but she could not rem..
doc: really? (den turn to madam Ho) so when did this pain start?
MH: oh, bout 1year ago..

OH MY GOD !!!
i wanted to slap her lor..
first cannot rem.. den doc come already..
suddenly memory so good..

doc: den for her pain.. is she on any medication for it?
kerly: nope, madam Ho is currently not on any medication
doc: what? did you ask her?
kerly: yes, she told me that she was not on any medication.. and has no drug allergies..
doc: (turn to madam Ho) is that true?
MH: no, i got eat the medicine you gave me.. and the laxative also..

WAH.. i nearly fainted..

doc: so tell me, wad were her aggravating and relieving factors?
kerly: the pain gets worst when she bends down to touch her toes..
doc: yes, somemore?
kerly: and it hurts sometimes when she lies down
doc: okae, den how bout the relievin factors?
kerly: there was no relievin factor.. she only avoids bending down..
doc: oh really? did you EVEN ask her bout it?
kerly: yes sir, i did..
doc: (turn to madam Ho) so got any relieving factors or not?
MH: got !!! when i eat the laxative you gave me.. den it makes me shit.. den less painful..

ARGH... i'm so so dead..
i could see that my doc was beginnin to get upset with me already..
he must be thinkin..
"this girl cant even take a decent history man!!!"

doc: so is her pain radiating? did it spread to other parts of her body?
kerly: yes, it spread from her left loin to her left lumbar and umbilical regions
doc: thats it?
MH: no, it even spread to my whole leg and toes.. very pain leh..

WAH.. i'm officially dead !!!!
the madam Ho has just sabo-ed my entire history taking..
and one more thing..

the room that i had to take history in, only had an examination bed and a chair..
so i told madam Ho to rest on the bed while i sit on the chair..
but she refused.. she said she would be more comfy sitting on the chair..
so she made me sit on the bed instead..
and when the doc came in..
he was absolutely horrified to see me sittin on the bed..
and he even gave me that look..
that "how dare you make the patient sit on the chair" look

oh horror upon horror !!!
i'm so gonna fail this exam !!!
i hate you, madam Ho !!!
